- Leadership assurance (ISO 45001): risk profiling, PDCA, KPIs, benchmarking

Element 2: Human Failure & Decision‑Making
- Error vs violation – skill‑based errors, mistakes
- Routine, situational, exceptional violations
- Automatic vs reflective decision making
